There is a cat ear option but those are fake and they come off when you accidently remove it. The power of modding reins supreme but I wish for a game where there more options you less have to mod the game because the customization options could have been more. Will there be a custom waifu game where you can make a real fox girl with live ears and tails. What if you are in to monster girls, do you wish for that feature or do you wish to find a mod to give that feature. Now speaking of those games, we have yet to see new title besides Honey Select 2 and AI Shoujo. Even that Koikatsu vacation game. Will there be a custom waifu game that offers you to make cat girls, fox girls, dragon girls, monster girls, and demon girls? Well, here is one condition that can make all that a possibility. However, this will take a new IP or an existing IP to make a specific themed spin-off.

    There is one condition and that condition is that if a new game takes place in fantasy world or an existing franchise gets a spin-off that gets the Isekai treatment. Going that route ensure more possibility find a build a waifu with monster girl, kemonomimi girl, and demon girl options. Of course, no build a waifu never leave out the elf ears option lately. It would be a no brainer if elf girl options carry on over. However, the fantasy world will give us the ability to as an example, make a fox girl with authentically fluffy and live ears and tails. The idea of a fantasy setting build a waifu is definitely going to bring forth the options. Here is the issue, it hasn't been made yet as we know of....

    Should the build a waifu simulator games continue to exist (we hope the fuck so..), we can more likely see a H-Game dev take on the isekai or fantasy setting. As of the time being, the cat ears, horns, tails, and etc is definitely fake but they are close enough. However, in the great power of modding, you can make it possible. But just like waiting on a game dev to bring one, it is the same song and dance in the mod scene. If we have to evolve another gaming generation, then let's hope for the best.
